The 7M engine (both GE and GTE variants) is prone to blown head gaskets due to Toyota's factory under-torque specification on the head bolts. The original spec of 58 ft/lbs is insufficient, causing the head gasket to work loose over time, wearing depressions into the head and block mating surfaces from the fire ring moving around.
Toyota made an error in the original factory torque specification, setting head bolt torque at only 58 ft/lbs and printing it in their service manuals. This insufficient clamping force allows the head gasket to shift over time, eventually causing failure. Overheating accelerates the failure.
