Clutch Not Engaging - Clutch Adjustment Was Way Off

Problem

Clutch would not engage to the flywheel, acting as if the pedal was always pushed down (disengaged). The clutch fork moved freely by hand and did not appear to be doing its job. Bleeding the slave cylinder resulted in fluid shooting out with the fork not responding to hydraulic pressure.

Root Cause
